Special Waste Disposal Checklist
(Use before shipment of any special waste load)

__ The load was checked to ensure that the following types of waste were not loaded.
  · RCRA hazardous wasteas defined by 40 CFR 261
  · Inhalable carcinogens (material known to cause cancer in human subjects)
  · Regulated medical waste, including live vaccines and unused sharps
  · Radioactive waste
  · Waste that contains >1% iodine
  · Waste with a halogen or sulfur content >5%
  · TSCA Regulated PCB waste
  · Asbestos containing waste
  · Cytotoxic waste
  · Bunker oil with Hazardous Constituents or sulfur >5%
  · Waste larger than a 55-gallon drum
  · Waste that contains >5% zinc or zinc compounds
Some special wastes are acceptable only under certain conditions, and may be limited in quantity, feed rate, and/or handling.
__ To prevent shifting or tipping over, waste has been packaged in suitable containers and tightly shrink-wrapped or banded to pallets.
__ Heavy weighted pallets or skids were not placed on top of lighter pallets or skids.
__ Suitable containers include cardboard boxes, plastic drums or fiber drums.
__ Plant's specific procedural list was checked for acceptability regarding steel drums. See "Special Waste Disposal Procedures.
__ All waste was clearly labeled to reflect the contents and is in conformance with DOT regulations.
__ Waste was labeled such that it matches the customer's approval report
__ Labels have the generator's name, address, type of waste, and approval number.
__ Labels for drummed waste must indicate whether the drums have powders.
__ An inventory list of the shipment is attached to the manifest or bill of lading.
__ A non-hazardous manifest (available from the Special Waste Coordinator) or bill of lading is with each shipment.
__ Non-hazardous waste manifests.
__ A licensed non-hazardous waste hauler must transport the waste.
__ The special waste delivery was scheduled at least 24 hrs in advance with the Special Waste Coordinator
__ The load was scheduled by calling 888-Ref-Fuel (888-733-3835), ext. 4965 or by e-mail, adennis@ref-fuel.com.
__ The following information was provided when the load was scheduled - customer name, generator name, account number, approval number, name of transporter, type of vehicle, amount of pallets/skids, witness loads (indicate amount of witnesses, direct feed or modified burn, elevator or pit burn), facility waste is being delivered to, and whether transportation needs to be arranged.
__ The Special Waste Coordinator was notified when the scheduled time could not be kept.
__ The driver and witnesses are aware of the driver guidelines and tipping floor safety requirements. See "Special Waste Procedures"
