Analytical instruments shipped from job to job without checkup or cleaning. Bench check instruments after each job with the instrument technicians card attached to the top. Ask to see the instrument log repair history file and bench calibration. Error should be identified with field calibration. Testing delayed by bad calibration.
Heated sample lines shipped from job to job without checkup or cleaning Flush HSL after each job then use CO and THC analyzers to confirm that the residual levels are less then 1 ppm. Contaminated blanks. Instrument grade reagents and new glassware for every reagent preparation. Lab and field blanks that are more than the sample.    
Negative sample weights. We pre-condition filters before use and select the filter media will not be attacked by the compounds present in the sample gas. Negative sample weights.    
Field delays and problems caused by test equipment failure. Rugged and dependable test equipment. Is it engineered to eliminate failure?  Is it well maintained? Equipment failure contingency plan. Possible bias error but bigger concern should be fabrication to replace lost data. Delayed testing. Prolonged process operation at test condition, such as reduced load,could be very costly. 
Lost of computer stored test data. Back-up all project-related computer files on a DAILY basis in field and office. Field computer backup disk which is stored off site. Ask to see office backup system. No bias error unless data is fabricated to replace lost data. Repeat of testing program may be needed but may not be possible in special situations.
